New Techniques Developed to Make Breast Enlargement Safer

Pacik, founder of the Plastic Surgery Center, presented a combination of three techniques developed over the past 4 ? years to make breast enlargement surgery safer and more effective at the 47th annual meeting of the New England Society of Plastic and Reconstructive Surgeons, Inc. in New Castle, New Hampshire (June 2nd through June 4th, 2006).Dr. Pacik presented his new techniques, which provide improved integrity to the breast fold, thereby reducing the complication of bottoming out in a paper titled Augmentation Mammaplasty: Managing the Inframammary Fold.

The three techniques used in combination are enhanced lowering of the inner lower (thicker zone) of the breast fold giving improved rounding and cleavage to this area while at the same time protecting the position of the implant. (This technique was previously reported in the Aesthetic Surgery Journal, Augmentation Mammaplasty: Enhancing Inferomedial Cleavage, Vol. 25, July/August 2005, pages 359-364.)Bottoming out is a potentially serious complication of augmentation mammaplasty because the implant migrates to an excessively low position. The tissues needed to repair this complication are very thin and complications such as pneumothorax and recurrence of the bottoming out can occur, says Dr. Pacik.

By respecting the integrity of the breast fold during the primary operation for breast enlargement, this serious complication can be reduced, and the operation made safer.Dr. Pacik combines these procedures with a muscular sling from the pectoralis major muscle to the level of the lower portion of the breast fold and blocking sutures to safeguard against bottoming out. Our incidence of bottoming out is about 2%, says Dr. Pacik. Other centers have reported this complication to be as high as 10%.
